En un formulario agrega el control flexgrid
A la barra de herramientas - Microsoft FlexGrid Control x.x (C:\Windows\System\MSFLXGRD.ocx)
Agraga al Form:
2 textbox
2 labels
1 commandbuton
1 flexgrid
